    Skip this step if you don’t have Medicaid. If you are a Medicaid recipient, you will need to go through the approval process for Long Term Care services. After you get approved for Long Term Care, you will get a case manager assigned through your designated Case Management Agency (The Resource Exchange, Rocky Mountain Human Services, or Developmental Pathways). This designated case manager will need to be involved in the process of coming on board with Allevia Home Care.

  2. Schedule a Home Assessment

    Give us a call at (720) 500-4444, or complete our short home assessment form. Our office staff will contact you to set up a care meeting. Upon meeting, we can discuss all you/your loved one’s care needs, and how best Allevia can serve those needs.
