How To Get Started ?

3 Easy Steps To Home Care

  1. Medicaid - Approved for Long Term Care

Skip this step if you don’t have Medicaid. If you are a Medicaid recipient, you will need to go through the approval process for Long Term Care services. After you get approved for Long Term Care, you will get a case manager assigned through your designated Case Management Agency (The Resource Exchange, Rocky Mountain Human Services, or Developmental Pathways). This designated case manager will need to be involved in the process of coming on board with Allevia Home Care.

  2. Schedule a Home Assessment

Give us a call at (720) 500-4444, or complete our short home assessment form. Our office staff will contact you to set up a care meeting. Upon meeting, we can discuss all you/your loved one’s care needs, and how best Allevia can serve those needs.

  3. Select The Right Caregiver

We personalize the caregiver selection process by matching you with someone who:

    • Has experience that aligns with your care needs
    • Fits well with your personality
    • Can accommodate the schedule you need
    • Who is kind and caring

If it’s not the right fit, just let us know and we will assign new staff immediately. Our goal is to have all parties involved comfortable and happy.
